Health Forms:

Class 1 Health Form:

Tto be used by all Cub Scouts and Leaders attending Cub Scout Day Camp, Bear Resident Camp, Webelos Camp and Boy Scouts and Leaders (under 40 years of age). This form is completed annually.

Class 2 Health Form:

To be used by all Scouts and Leaders (under 40 years of age) attending Boy Scout Camp. This form is good for 36 months.

Class 3 Health Form:

To be used by all Leaders (over 40 years of age) attending Boy Scout Camp and ALL Camp Staff Members. This form is completed annually.
